To save Greirat from Irithyll, you have to save Siegward from the well outside the Church before the Cathedral of the Deep by buying his armor from Patches and giving it to him. Before sending Greirat to pillage Irythill, I ran into Patches and got him to stay at Firelink, saved Siegward and gave him his armor whilst buying another set off of Patches (NG+). Then AFTER I cleared Irythill up-to the Church of Yorshka bonfire I homeward bone'd back to the Shrine and told Greirat he is good to go.
